What is the minimum clearance to doors and windows for an outdoor tankless gas water heater?

The vent termination of an outdoor (not direct vent) gas tankless water heater should not be within an area extending four feet below or to the side, and one foot above, any door or the operable area of a window that can be opened. The water heater shown above is clearly not in compliance. We think that the reason this sometimes happens may be that the installer is going by the specs for installation of a direct vent (indoor) tankless water heater, which allows the vent termination to be within one foot of a door or openable window.
These clearances were taken from a Rinnai installation manual, which references ANSI A223.1/NFPA 54 for the standards.
